Important Documents

Employees must provide the Employment Accommodations Resource Center (EARC) with written, medical verification of a mobility impairment or functional limitation that substantially impacts ambulation. To be eligible for parking an employee must provide a medical diagnosis, which must be determined by a licensed physician and submitted on professional letterhead. The diagnosis must be based upon history and treatment for a medical condition that adversely affects mobility.

Documentation must include an evaluation of the impact on the individual of both the diagnosed condition and any prescribed medications. This evaluation must include an explanation of the functional limitations of the disability and address how the impairment substantially limits the individual’s major life activity of walking. Indicate whether condition is short-term (less than 90 days), or long-term (more than 90 days).

DMV Placards

Employees with a California state-issued DMV placard must also display a valid SJSU parking permit to park in any handicap parking space on the SJSU campus.

Parking Lot Key Cards

A key card is necessary to gain access to Lot #1 located at 7th & San Fernando. To apply for a key card, employees must submit an application to Traffic and Parking Operations located in the 7th Street Garage. If the electronic gate malfunctions, you may contact the EARC by dialing 111 during business hours on the Lot#1 gate control key pad or use the gate control directory to contact the University Police Department (UPD) to gain access.
